OPERATION AND MAINTENANCE PLAN <br />Water Conveyance Facili:y: L. E. D. E. RESERVOIR & DitCh <br />Easement Solder : TOTniri Of Gyj~SUm <br />Tnis Special Use Permit incorporates this Operation and Maintenance Plan (0 & M <br />Plan) and any attachmsn=s thereto; and the Holder agrees to operate and maintain <br />the facilities and usz Lhe Special Use Permit in accordance with the following <br />stipulations; <br />1. CJse only approved equipment for rputine operations and rnaintenance as lis•ted <br />below, travel only the ataintenance routes agreed to and repair all damaqe resulting <br />from szid use. <br />Vehicles/ Equipment: <br />ATVs-rou~ine inspections & operations; Backhoe/excavator-routine ditch <br />cleaning <br />• Holder must have a copy of this 0& M Plan with them at a11 times when <br />driving motoriz~d vehic~es along d_tch banks or any access routes that are <br />not open for motorized use by the general public. A copy this 0&M Plan shall <br />serve as authorization for motorized travel off designated routes. <br />• Use of these roates is authorizzd for water facility operations and <br />maintenance purposes on1y. • <br />• Aolder is subjecL to federal and state laws regulating travel of State, <br />County and Federal lands; including Code of Federal Regulations CFR 261.15(h) <br />which nrohibits travel in a manner which damages or unreasonably disturbs <br />land, wil.dlife, or vegetative resources. <br />• The holder of this facility is autharized to aecess this fzcility for <br />routine operat~ons and maintenance via the ditch banks and the identified ATV <br />access route off NFST 1872. <br />2. The fiolder sha11: be responsible for pre~ention and control of soil erosion and <br />gullyinq on land covered by the easement and the land adjacent thereto, resulting <br />from opezations and maintenance of qran~ed use;inspect diversion and heac4gate <br />structures to ensure propex functiona.nq; maintain channel to prevent dorm cutting <br />and bank failure; remove all ~bstructions Erom the channel; revegetate or otherwise <br />stabilize all ground where the soi}. has been exposed; be responsible for control of <br />and spread of noxious weeds, as identified by the US Forest Service and the local <br />County weed list. <br />3. The Solder shall inspect the facility Qrior to use each year, periodically <br />during the aperating season and after all major storms or major flow events and <br />make necessary repairs. Work tha~ is considered other than routine maintenance <br />and/or minor xepairs shall be discussed in advance with the Forest oEficer. A11 <br />repairs shall be acceptable to and completed by the date agreed to by the Holder <br />and the Forest officer. <br />9. Tha Holder will contact the Fcres~ officer for approval before proceeding with <br />work that is other than routine operations. Some of these situations are: <br />a. Bringing in and using heavy equipment for use other than routine or <br />eme~gency maintenance of the existing structures. <br />b. Using other than approved ma?ntenance routes for access. <br />
