2. Dam Classificatian. <br />The dam cor~structed pursuant to this authorization shall be ~lassif~ed according to its height and storage capacity <br />(water debris or both) as well as its hazard potential as follows: <br />Height and Starage Capacity (A, B, C, ar D): class B- 45` height and 473 acre feet storage <br />capaciry <br />Hazard Potentlal {Low, Moderate, High}: Moderate <br />Classification criteria are contained in FSM 7511, which the Forest Service may amend frorn time to time. <br />The provisions of sections 5 and 8 oF this clause apply only to dams classified as high hazard, or as atFterwise <br />may be specifically provided for in this authorization to address special or unique c'srcumstances. <br />The hazard potential of the dam shall be reassessed at least every five years by a qualified engineer retained by <br />the hofder,.and this infarrnatian made available Fo the authorized o~cer. 7he Forest Service may change the <br />hazard potential at any time based on changed conditians or new infnrmation. <br />3. Construction, Inspection, Certificatian, and Project Files. <br />Fo~ construc~on, rehabilitatpn or improvement, the holder shall provide for inspection by a qualified engineer to <br />ensure adequate control of the work being performsd. At a minimum, the qualified engineer shall maintain a daily <br />inspection diary, descriptions of design ettanges, and recards of construction material and foundation tests. <br />Upon completion of construction, rehabi(itation, or improvement, the holder shall fonuard to the Farest Senrice a <br />statement #rom the qualified engineer responsible for inspectian certifying that the works were built in accordance <br />with the approved plans and specificatians, or approved revisions thereto. iVo water shatl be impounded until <br />approval is given by the authorized officer. <br />All design notes, as-built plans, and the aforementioned dlaries and records sha11 be maintained in a project file by <br />the hotder for the dwration of this authorization, and shall be available to the Forest Sarvice or other inspectian <br />personnel (not applicable to debris retention dams}. <br />4. Dam Operation and Maintenance Pians. <br />Darn operation and maintenance plans shall be prepared during the design phase for new dams. The plan(s) <br />shall, as a minimum, describe operating requirements and procedures to be folfowed for the operaiion of the <br />structure; routine ar recurring maintenance required; record keeping ta kie pe~formed for operation and <br />rnaintenance; and individuals responsible for implemer~ting the plans. At the time of the operation and <br />maintenance inspection, the plan shall be reviewed and amended as needed by the individua{ res.ponsible for <br />implementation and iha engineer pertorming any inspection. No plans or amendments thereto shall be valid until <br />approved by the auEhorized officer. <br />5. Darn Emergency Action P(an. <br />The fallowing provisions are required for certain hazard classifications identified in section 2. The holder shall, <br />during the design phase, prepare an emergency ac6on pian which wilf include, but nat be (imited to: <br />a. Actions to be taken upon discovery of an unsafe condition or impending failure situation to prevent or <br />de(ay dam failure, and reduce damage or loss of life frorr~ subsequent failure, <br />b. Procedures For notification of 4aw enfarcement, civil preparedness, and Forest Service personnel. <br />c. Procedures for notifying persons !n immediate danger of losing life or praperty. <br />d.
