DRAFT PLATTE RIVER RECOVERY IMPLEMENTATION PROGRAM REQUEST FOR PROPOSALS SUBJECT: REQUEST DATE: CLOSING DATE: Insurance Procurement October 31, 2007 November 30, 2007 POINT OF CONTACT: Dr. Jerry Kenny Executive Director Platte River Recovery Implementation Program 3710 Central Avenue, Suite E Kearney, NE 68847 (308) 237-5728 kennyj cr,headwaterscorp.com RECITALS The Governance Committee of the Platte River Recovery Implementation Program (Program) submits this Request for Proposals (RFP) to solicit proposals to obtain insurance coverage as outlined herein for the Governance Committee representatives and subcommittee representatives for certain actions that will be undertaken through Program implementation. The successful policy should have a limit of at least $10 million per occurrence and be by an "A-rated" insurer. The attached description of the Program should be considered before developing the proposal. In responding to this RFP, the Governance Committee requests the following information: 1) A statement of qualifications. At a minimum this should include a description of the company, including, the name, address and phone number of the contracting officer and any other important personnel, the number of years the company has been in business, and types of insurance provided to other entities. 2) Scope of coverage/Description of the Policy. Prospective contractors should provide a detailed description of the coverage they are proposing to provide. At minimum the scope of coverage should include coverage for the Governance Committee representatives and subcommittee representatives and the organizations that employ the representatives, for certain actions that will be undertaken through Program implementation, including the intentional release of a pulse flow to the Platte River. The insurance should cover, at a minimum, General Liability, Property, Bodily Injury, and Management Liability. Each policy should include coverage of $10 million per occurrence, but any higher coverage may be included in the proposal also. Several of the organizations represented on the Governance Committee (CNPPID, NPPD, others) will have to be listed as co-insured. Coverage should apply regardless of any limitation of liability otherwise available to the insured parties (e.g. sovereign immunity or government tort claims acts). The proposal should be clear on who would be the insured, and on any specific conditions that would be part of the policy.
