Water Supply Reserve Account - Grant Application Form <br />Form Revised May 2007 <br />Task 1 <br />Please summarize the proposed scope of work. Please refer to Part 2 of the Criteria and Guidance document for <br />detailed requirements. On the following page there is an example format for the Scope of Work. You can use <br />the example format or your oval forinat, provided that comparable information is included. <br />The scope of work should outline by task how the water activity- will be accomplished. It is important that the <br />scope of work detail the specific steps, activities/procedures that will be followed to accomplish the water activity <br />and the specific products/deliverables that will be accomplished. The scope of work should include but not be <br />limited to: task description, key personnel, budget, schedule and deliverables and the final report/project <br />documentation upon completion of the water activity. <br />Drill commercial well <br />Well permit obtained <br />Augmentation agreement made with Pine River Irrigation District for water <br />Easement obtained for well and treatment building <br />Well drilled by Campbell Drilling to a depth of 430' <br />Water tested for both quantity and quality and found to be adequate <br />(see attachments for Section B.6.) <br />Schedule: <br />• completed Spring 2008 <br />Task 2 <br />Design Report <br />Detailed report submitted to the Colorado Department of Public Health and Environment showing <br />the svstem design and the systems ability to complete the upgrades both managerially and <br />financially. <br />Design Report to by completed by Harris Water Engineering <br />Design Report review and acceptance by State required <br />Schedule: <br />Expected completion and submittal date is Sept. 15th 2008 <br />State acceptance expected within 45 days of submittal, October 30th <br />Task 3 <br />Treatment building <br />Size: 20'x16' <br />Contains: Pressure tanks, pump, chlorination equipment, backup generator, distribution lines <br />The treatment building will be built by Quentin Wray, Inc. with help from member volunteers. <br />Schedule: <br />• The foundation, frame and roof have been completed. <br />• The treatment building will be completed after the Design report is accepted by the State and <br />funding becomes available. One week will be sufficient time to complete the building. <br />8
