blended source, to the customers who are presently served directly from the North Springs <br />delivery main. This is required to brine the entire notable water system into compliance. <br />Please summarize the proposed scope of work. Please refer to Part 2 of the Criteria and Guidance <br />document for detailed requirements. On the following page there is an example format for the Scope <br />of Work. You can use the example format or your own format, provided that comparable <br />information is included. <br />The scope of work should outline by task how the water activity- will be accomplished. It is <br />important that the scope of work detail the specific steps, activities/procedures that will be followed <br />to accomplish the water activity- and the specific products/deliverables that will be accomplished. <br />The scope of work should include but not be limited to: task description, key- personnel, budget, <br />schedule and deliverables and the final report/project documentation upon completion of the water <br />activity. <br />1. Task 1 <br />Description of Task: To Install a `Backfeed" Potable Water Distribution Main which will deliver blended <br />potable water from the existing potable water storage tanks, located in Town, to the 39 taps which were <br />previously identified. <br />Method or Procedure to be followed to accomplish the task and identifi- who will undertake the task: First <br />the Preliminary Engineering Report (PER) shall be completed by Young Technology- Group <br />(YTG)(Engineer). Secondly, submit the PER with funding application to the various fiinding agencies <br />(Town of Fowler). Thirdly, procure funding (Town of Fowler). Fourth, commence field surveying <br />operations (Bnrndridge Land Surveying, Inc.). Fifth, commence Design Phase(YTG). Sixth, obtain approval <br />of plans and contract documents by Funding Agency-(YTG). Seventh, let project for bid (YTG/Fowler). <br />Eighth, Award contract to lowest responsible, eligible Bidder(FoNvler). Ninth, quality control and project <br />oversight (YTG). Lastly, final inspection, acceptance and payment to Contractor, less retainage and warranty <br />(YTG/Fowler). <br />Description of the major deliverables/products that will be produced upon successful completion of the task: <br />The major deliverable shall be the underground pipeline. All O&M material related to all aspects of the <br />svstem, shall be delivered to the Town of Fowler, by the General Contractor. <br />Young Technology- Group (Resume Attached) <br />INSTALLATION OF A POTABLE WATER DISTRIBUTION MAIN FROM TOWN TO THE <br />TAPS WHICH ARE PRESENTLY SERVED DIRECTLY FROM THE NORTH SPRINGS <br />Cost Estimate for Installation of 10,200 Lineal Feet of 6" Diameter <br />Potable Water Main Backfeed Line to Existing Costumers <br />Prepared bye: Young Technology Group - 11/29/2007 <br /> <br />uantit?
