Water Supply Reserve Account – Grant Application Form <br />Form Revised May 2007 <br />_______________________________________ <br />Field control survey to locate existing spring and improvements, existing waterline <br />& improvements from spring down into and thru town. <br />Location of existing buildings and improvements on or near the existing waterline. <br />Prepare a site plan showing locations and topographic information along the <br />waterline route. <br />2. Preliminary Engineering for designing the distribution system ($11,082). New system will <br />be expanded to include all platted lots within the Town. <br />Design will include looping water mains to improve flow and pressure as well as <br />delivery reliability. Design will ensure that all mains are on public rights of ways. <br />Model Distribution system to ensure good pressure and flow distribution. <br />Develop base mapping of existing system <br />Using model results and base mapping, deve lop a new distribution system, including <br />valves, blow off, etc, for enhanced operation of system. <br />Develop preliminary construction drawings and details <br />Prepare quantity schedule along with engineers opinion of cost. <br /> 3. Final Engineering will include final plans and related documents for use by Sapit in selecting <br />a construction contractor and s eeking implementation funds ($8,918) <br />The estimated cost of construction for the project is $60/linear foot or between $300,000 and <br />$600,000, not including a 25% for contingency, plus anot her 10% for mobilization costs. Prior to <br />completing the survey, which will determine the exact le ngth of distribution lines, it is difficult to give <br />a more specific construction cost estimate. <br />5. Please summarize the proposed scope of work. Please refer to Part 2 of the Criteria and Guidance document <br />for detailed requirements. On the following page there is an example format for the Scope of Work. You can <br />use the example format or your own format, provi ded that comparable information is included. <br />The scope of work should outline by task how the water activity will be accomplished. It is important that the <br />scope of work detail the specific steps, activities/pr ocedures that will be followed to accomplish the water <br />activity and the specific products/deliverables that will be accomplished. The scope of work should include <br />but not be limited to: task description, key personne l, budget, schedule and deliverables and the final <br />report/project documentation upon completion of the water activity. <br />A scope of work from Buckhorn Geotech is attached: <br />1) Estimate Letter for Surveying – Please note that only Task 1 of the proposal is relevant to this project <br />2) Scope of work/proposal for Engineering <br />7 <br />
