Box 435
Frederick, Colorado 80530

Re: Milavec Lake Seep Prevention and Collection Alternatives; State Dam ID 055319

Dear Charlie:

Rocky Mountain Consultants, Inc. (RMC) is pleased to submit this revised proposal to provide engineering
services for evaluating seep prevention and/or collection alternatives for Milav.ec Lake. RMC has incJuded
a proposed scope of work and associated engineering costs for the site investigation and lining alternatives
evaluation for Milavec Lake.

Enclosed, please find two copies of our standard Professional Services Agreement to provide services as
described below.

Task 1 - Existinl! Data Collection and Reservoir Water Balance

RMC will collect available geotechnical and hydrologic data including existing boring log information and
perform a water balance from existing information to ascertain potential problem areas of the reservoir
where the majority of the leakage may be occurring. RMC will construct maps and cross-sections based on
available information. Task I will provide the background information and technical basis for completing
the Task 2 Field Investigation.

Task 2 - Preliminarv Field Investil!ation

RMC will complete an on-site field investigation based on data gaps or deficiencies identified in Task 1.
RMC estimates that the on-site investigation will include installation of approximately four to eight
piezometers or well points. RMC will apply for the necessary permits from the State Engineer's Office for
installing the piezometers and contract a drilling company to complete the piezometers. RMC estimates
that the drilling program for the preliminary field investigation will require 20 hours of drill rig time.
Samples will be collected at select intervals for future laboratory testing as necessary. Additional drilling
and soil testing may be necessary after a design alternative has been selected. ~

RMC will also visually observe the dam embankment and adjacent areas for possible loss areas from the
reservoir. Water volume and specific conductance measurements will be completed in thestormwater
ditch which runs adjacent to the reservoir along WCR 13 to ascertain whether this ditch may be a major
seepage loss source for the reservoir. Tasks 1 and 2 will provide the technical basis for providing the most
cost effective seepage collection and/or lining alternatives.

CIVIL AND ENVIRONMENTAL ENGINEERING.
