<br />Water Supply Reserve Account - Grant Application Form <br />Form Revised April 2007 <br /> <br />Refer to Attachment #2 "Evaluation Criteria" for a detailed response to the <br />"Evaluation Criteria", items "a" through "t". <br /> <br />4. Please provide an overview of the water project or activity to be funded including - type of activity, statement of <br />what the activity is intended to accomplish, the need for the activity, the problems and opportunities to be addressed, <br />expectations of the participants, why the activity is important, the service area or geographic location, and any relevant <br />issues etc. Please include any relevant Tabor issues that may affect the Contracting Entity. Please refer to Part 2 of <br />Criteria and Guidance document for additional detail on information to include. <br /> <br />For this information, please refer to: <br />---"Project Summary Letter" <br />---"Preliminary Engineering Report" (see "Project Summary Letter" as how <br />to access the Preliminary Engineering Report. <br />---"Attachment #2 "Evaluation Criteria" <br /> <br />5. Please summarize the proposed scope of work. Please refer to Part 2 of the Criteria and Guidance document for <br />detailed requirements. On the following page there is an example fonnat for the Scope of Work. You can use the <br />example format or your own format, provided that comparable information is included. <br /> <br />The scope of work should outline by task how the water activity will be accomplished. It is important that the <br />scope of work detail the specific steps, activities/procedures that will be followed to accomplish the water activity <br />and the specific products/deliverables that will be accomplished. The scope of work should include but not be <br />limited to: task description, key personnel, budget, schedule and deliverables and the final report/project <br />documentation upon completion of the water activity. <br /> <br />Please refer to the Preliminary Engineering Report, Environmental Report and Archeological <br />Report, (accessed on-line) for detailed information. <br />A Summary of major project milestones are as follows: <br />--Collect & Escrow Tap Fee; fall 2007; GPW A; MWC <br />--Sign Contract with Rural Development; fall 2007, GPW A; RD <br />--Develop Operating Agreement; Winter 2007-2008; GPW A; MWC <br />--Develop Request for Proposals; Winter 2007-2008; Briliam Engineering <br />--Review and Award Bid; Winter 2007-2008; Briliam Engineering; GPW A <br />--Acquire necessary Right of Way Easements; Winter 2007-2008; Briliam <br />Engineering; GPW A; Montezuma Water Company <br />-- Sign Contract with successful contractor (SC);Spring 2008; GPW A; SC <br />--Obtain construction permits; spring 2008; SC <br />--Begin construction; spring 2008 to November 2008; SC <br />--Construction monitored by MWC; RD; GPW A; Permitting Agencies <br />--Construction complete, tinal test of system; November 2008;SC, MWC <br />--System "turn over" to Montezuma Water Company-begin operation; <br /> <br />5 <br />
