deficiencies within twenty days after receipt of the application and shall <br /> allow the applicant an additional fifteen days to submit the necessary <br /> information, fee, or signature. If the applicant fails to remedy the <br /> deficiencies within fifteen days, the State Engineer shall return the <br /> application and the application fee shall be forfeited. <br /> Rule 7. Information to be Submitted After a Well Permit is Issued: <br /> A. If a well is completed for which a valid permit exists, the <br /> permittee shall submit prior to the expiration of the permit the following <br /> information on a form prescribed by the State Engineer, together with such <br /> other information as is required as a condition of approval on the well <br /> permit: <br /> 1) The permit number; <br /> 2) The name, mailing address, and phone number of the applicant; <br /> 3) The well location as constructed by identifying the section, <br /> township, range, and prime meridian, and the well location as constructed <br /> in feet from the nearest north/south section line and from the nearest <br /> east/west section line, or, in areas of the state that are not surveyed, <br /> an alternate description of the well location sufficient to locate the <br /> well; <br /> 4) The date completed; <br /> S) The name and license number of the water well contractor; and <br /> 6) Any other information, including, but not limited to, the <br /> results of specific yield tests, that . is required as a condition of <br /> approval on the permit, except that such information shall not be required <br /> to be submitted prior to the expiration of the permit if another <br /> submission date is stated in these rules or in the permit. <br /> B. The licensed water well contractor and/or pump installer shall <br /> submit to the State Engineer the information required by Section <br /> 37-90-138, C.R.S. and Sections 37-91-101 to 112, C.R.S. , the information <br /> required by the Rules and Regulations of the State Board of Examiners of <br /> Water Well Construction and Pump Installation Contractors, any information <br /> required of the water well contractor or pump installer on the permit, and <br /> any other information required by applicable laws or rules and regulations. <br /> C. The permittee shall submit to the State Engineer a Notice of <br /> Commencement of Beneficial Use on a form provided by the State Engineer <br /> within thirty days of the first beneficial use of any water withdrawn from <br /> a well permitted pursuant to Section 37-90-137(4), C.R.S.
