Pursuant to CDPHE WQCD request, submitted 7/30/97, DMG conducted a final
inspection of the Eureka Tailings Mine Remediation Project. The Eureka Tailings area is
located at the confluence of Eureka Creek and Animas River, in portions of the SE 1/4 of
Section 19, T42N, R6W, NMPM, San Juan County.
Inspection Date: 8/5/97 '__� Inspection occurred from 2:00 pm - 3:30 pm.

Field Conditions: Overcast and warm, intermittent rain.
Observations: Reclamation backfilling and grading appeared to have been completed in
accordance with conditions of the Consent Decree and in accordance with the Colorado
Mined Land Reclamation Act and the Hard Rock/Metal Mining Rules and Regulations of
the Colorado Mined Land Reclamation Board. Pre-law tailings from the Eureka Mill site
had been relocated to Mayflower Pond 4. Pockets of additional tailings, not originally
identified within the Consent Decree, were discovered and relocated to Mayflower Pond
4. Relocated materials were neutralized with high pH material at Pond 4. Limestone
materials were also incorporated into excavated areas at Eureka during backfilling and
grading of affected areas. Affected areas at Eureka were seeded and mulched.
Approximately 60 tree seedlings and 60 shrubs were transplanted. Spruce and limber
pine seedlings and various deciduous shrubs were evident. Metal debris and discarded
equipment were removed from the Eureka site. Seeding, mulch, and tree and shrub
transplants were not required by the Consent Decree. DMG appreciates operators
commitment to reclamation of affected areas beyond the minimum requirements.
