Pineda <br />Director <br />On July 30, 2012, the Division of Reclamation, Mining and Safety (Division) received your 112 <br />Construction Materials Reclamation Permit Conversion Application package for the T Bone Stone <br />Quarry, File No. M- 2004 -030. Preliminary review of the application package determined that the <br />following items must be received before the Division can consider your application as being submitted <br />and technical review can begin: <br />1. The Division requires the company name listed throughout the application (including within the <br />notarization) match the company name registered with the Colorado Secretary of State. The Applicant <br />has listed the company name as `T Bone Stone, Inc.'. The company is registered (copy of registration <br />enclosed) with the Colorado Secretary of State as `T -Bone Stone, Inc.'. Please correct the highlighted <br />portions of the original application (enclosed) on a new application. In addition, affix a corporate seal, <br />as required on page 8 of the application. If there is no corporate seal, then write in `no seal'. <br />2. The coordinates provided for the Primary Mine Entrance Location on page two of the Application <br />Form appear to be located too far south of the mining operation. In addition, the entrance coordinates <br />differ from those which were provided for the existing 110c Reclamation Permit. Please indicate if <br />the location of the primary mine entrance will change under the 112c Reclamation Permit. If the <br />location will remain the same, then please revise the coordinates to match those which are currently <br />on file. The coordinates on file for the primary mine entrance are: N 40° 15' 39.88 ", W 105° 16' <br />4.71". <br />3. The Applicant has submitted a single map to serve as both the Mining Plan Map and Reclamation <br />Plan Map. The Mining Plan Map and Reclamation Plan Map must be separate maps. As required by <br />Rule 6.4.6, please submit a Reclamation Plan Map which shows the proposed topography of the area <br />with contour lines of sufficient detail to portray the direction and rate of slope of all reclaimed lands. <br />Please note that all maps must include the following: <br />Office of Office of <br />Denver • Grand Junction • Durango Active and Inactive Mines <br />
