Mullet Excavating, <br />LLC has followed the prescribed procedures as outline in Rule 1.6 of the Mineral Rules <br />And Regulations Of The Colorado Mined Land Reclamation Board For The Extraction Of <br />Construction Materials. The operator has completed all of the requirements and enclosed <br />with this transmittal the required documents for the Division of Reclamation, Mining and <br />Safety File Number M -2000 -115. <br />The applicant/operator has notified the Las Animas Board of County Commissioners and <br />the Soils Conservation District Office in Las Animas County. The Certified Return <br />Receipts are attached. The approved Notice signage was Posted at the existing site entry <br />gate. The affidavit is included in the submittal. The application was placed at the Las <br />animas County Court House in the Clerk and Recorders office please attached receipt from <br />Lynell Mazza Chief Deputy County Clerk and Recorder. <br />The applicant/operator published the PUBLIC NOTICE in the "CHRONICLE- NEWS" the <br />newspaper that is the main local /regional with a wide circulation in the surrounding area. <br />The Notarized Proof of Publication showing the dates published is in this submittal for <br />your file. Also included in this submittal is the Ad Text as it was in print under the legal <br />notices. The Notice was published four (4) consecutive weeks, once a week. <br />The Owners of Record of the surface and mineral rights of the affected land and the <br />Owners of record of all land surface within 200 feet of the boundary of the affected lands, <br />in this instance it is the same landowner, where contacted by Certified Return Receipt Mail <br />see the enclosed copy of the Return Receipt for the USPS. <br />
