Preliminary review of the information received determined that the following <br />items must be received before the Division can consider your application as being submitted and <br />technical review can begin: <br />APPLICATION FORM: <br />1.) The Division's files indicate the original permit area was 30.06 acres. The Division released 8.31 <br />acres during the previously submitted acreage reduction (AR02) leaving 21.75 acres in the current permit <br />area. Item #3 on the application form indicates the current permit acreage is 21.73 acres. Please submit a <br />new complete copy of the first page of the application with the correct permit acreage indicated for Item <br />#3. With the addition of 12 acres, the new total permit area should be 33.75 acres. <br />2.) An applicant must initial all the "Responsibilities as a Permitte" items listed on Page 5 and 6 of the <br />application form. Please submit a new copy of these pages with each item initialed. <br />3.) The applicant is required to post a notice sign at the entrance to the site as described in Rule 1.6.2 <br />(1)(b). The applicant submitted a copy of the example notice filled out with the appropriate information; <br />however, the certification section was not signed by the applicant indicating the notice sign was actually <br />posted. Please submit a signed certification page and ensure the notice sign is in place. However, the applicant shall clearly <br />describe where in the original application and /or supporting documents the information not included in <br />the amendment application, but necessary to render the amendment technically adequate, may be found. <br />Some of the Exhibits discussed below were not referenced on Page 2 of your submittal which indicates <br />the Exhibits you believe have not changed from the original permit application.
