Please respond to the issues noted below as soon as <br />possible. <br />Please expand the text narrative information provided as required by Rule <br />2.05.3(6)(a) to include the information for; <br />(i) types and approximate amounts of explosives to be used for each <br />type of blasting operation <br />(iv) description of types, capabilities, sensitivities, and locations of use of <br />any blast monitoring equipment and procedures proposed to be used. <br />(v) a description of the plans for recording and reporting to the Division <br />the results of pre-blasting surveys <br />2. Rule 4.08.1(4) requires the submittal of an anticipated blast design signed by <br />a certified blaster be submitted with the revision- application or the submittal <br />of a time for the design to be submitted before the blast. Please- p-r vide-the <br />required blast design or provide a time when this information will be - <br />submitted as a minor revision to the permit. <br />3.
