Pineda <br />Kiowa, Colorado 80117 Director <br />Re: Belveal Pit, File No. M-2001-056, Receipt of an incomplete 110 Construction Materials Reclamation Permit <br />Application Package AM-0\ <br />Dear Mr. Ehmann, <br />On March 10, 2011, the Division of Reclamation, Mining and Safety received your 110 Construction Materials <br />Reclamation Permit Application package for the Belveal Pit, File No. M-2001-056. Preliminary review of the information <br />received determined that the following items must be received before the Division can consider your application as <br />being submitted and technical review can begin. <br />APPLICATION FORM: <br />1.) The purpose of the amendment is to include the access road into the site has part of the permit area. Along <br />with the amendment application, the County submitted a release request for approximately .94 acres of a <br />portion of the current permit area. The Division will process this release prior to the approval of the <br />amendment, so the current permit area will be 8.96 acres. The release was done so that the County could <br />submit this amendment to add the.94 acres back into the permit area. However, item 3 of the permit <br />amendment application requests to decrease the permit acreage by.94 acres. The application should indicate <br />the following: <br />a. The permitted acreage (existing) should be 8.96 acres <br />b. The change in permit acreage should be +.94 acres <br />c. The total acreage in the permit area should be 9.9 acres. <br />Please submit a new cover page of the amendment application with this information corrected. I have <br />enclosed a new cover page with this letter. <br />EXHIBIT E - Map (Rule 6.3.5): <br />2.) In addition to the requirements of 6.2.1(2), you must provide a map that clearly describes the features <br />associated with the Reclamation Plan. Please submit one map for the Reclamation Plan. The map must be <br />drawn to a scale no smaller than appropriate to clearly show all elements that are required to be delineated by <br />the Act and these Rules; show a north arrow, note any section corners adjacent to the proposed operation, and <br />indicate the date illustrated. At a minimum, the map must include the following information: <br />a.
