Pineda <br />Director <br />The Division of Reclamation, Mining, & Safety (Division) received the application for <br />Amendment AM-05 on 13 January 2011. Our review indicates that the application is <br />incomplete. The following additional information must be provided before the application may <br />be designated as complete and filed: <br />An amendment application is considered filed when the application contains all information, <br />attachments, maps and exhibits listed in Subsection 1.4.1 of the Construction Materials Rules & <br />Regulations. Because the Amendment application requests the addition of acreage, the <br />application must include the following: <br />1) Exhibit A - Legal Description of the new area to be added. <br />2) Exhibit C - Pre-mining and Mining Plan Map(s) of Affected Lands for the new area to be <br />added. <br />3) Exhibit D - Mining Plan for the new area to be added. <br />4) Exhibit E - Reclamation Plan for the new area to be added. <br />5) Exhibit F - Reclamation Plan Map for the new area to be added. <br />6) Exhibit L - Updated Reclamation Costs including the new area to be added. <br />7) Exhibit S - Permanent Man-made Structures. Provide a list of permanent man-made <br />structures around the new area to be added, include the structures in the Exhibit C <br />map, along with the other requirements of Exhibit S and Exhibit C pertaining to <br />permanent man-made structures. <br />Applicant has 60 days from the date of this letter (01 April 2011) to provide the missing <br />information per Rule 1.4.1(8) or the application may be denied. After all of the <br />aforementioned requirements have been met, the Division will designate the application as <br />complete and filed, and technical review will commence. <br />Additional Considerations: Be advised that, after the application has been designated as <br />complete and filed, applicant must comply with the public notice requirements described in <br />Office of Office of <br />Minarl I and Rarlamatinn Denver • Grand lunction • Durango Active and Inactive mines
