Recia-rivAon, <br />iMInIM and Safety <br />Re: Hogue River Pit, Permit No. M-1992-066, Receipt of Construction Materials Regular (112) Operation Reclamation <br />Permit Amendment Application Package (AM2) <br />Dear Mr. Humphries: <br />On Friday, December 10, 2010, the Division of Reclamation, Mining and Safety received the Construction Materials Regular <br />(112) Operation Reclamation Permit Amendment Application (AM2) package for the Hogue River Pit, Permit No. M-1992- <br />066. A preliminary review of the information received determined that the following items must be received before the <br />Division may consider the application submitted and technical review can begin: <br />RULE <br />6.2.1 <br />6.4.17 <br />6.4.18 <br />General Requirements <br />(2)(b) (Maps) must be prepared and signed by a registered land surveyor, professional engineer, or other <br />qualified person; <br />Please sign and resubmit the enclosed maps. <br />EXHIBIT Q - Proof of Mailing of Notices to Board of County Commissioners and Soil Conservation District <br />Provide proof that notice, of the permit application was sent to the Board of County Commissioners pursuant <br />to Subparagraph 1.6.2(1)(a)(ii). <br />EXHIBIT R - Proof of Filing with County Clerk and Recorder <br />Provide an affidavit or receipt indicating the date on which the application was placed with the local County <br />Clerk and Recorder for public review, pursuant to Subparagraph 1.6.2(1)(c).
