The Division is returning Succession of Operator's Application <br />and Performance Warranty for the following: <br />Succession of Operators Application: <br />The Division requires completion of the Notary for Permittee section on page 5. Please have your notary list a title <br />and company name for Richard T. Heard. <br />The $259.00 annual fee for this site was due on July 26, 2010 and must be paid prior to approval of this application. <br />Performance Warranty: <br />The Division requires a date be listed for Mark T.
