Please <br />be advised that if you are unable to satisfactorily address any concerns identified in this <br />review before the decision date, it will be your responsibility to request an extension of the <br />review period. If there are outstanding issues that have not been adequately addressed <br />prior to the end of the review period, and no extension has been requested, the Division will <br />deny this application. <br />1.6.2 General Applicant Procedures <br />1. In accordance with Rule 1.6.2(1)(g), the Applicant must submit proof of publication and <br />proof of all required notices. Prior to approving the Amended Application, the Division must <br />receive documentation showing the Applicant republished a new notice of the application <br />and served copies of the notice to all Owners of Record of the surface and mineral rights of <br />the affected land and to the Owners of Record of all land surface; within 200 feet of the <br />boundary of affected lands. Proof of Notice may be by submitting return receipts of a <br />Certified mailing or by proof of personal service. <br />6.3.5 Exhibit E - Map <br />2. The Mining and Reclamation Plan Maps do not depict the entire permit boundary. The <br />access/haul road must be included in the Mining and Reclamation Plan Maps as it is a part of <br />the permit boundary.
