Dear Mr. Mullett:
On September 17, 2009 the Division of Reclamation, Mining and Safety approved the Amendment application submitted
to the Division on May 20, 2009, addressing the following:
Addition of 10.31 acres to the permit area for a total of 23.31 acres.
The terms of the Amendment No. I approved by the Division are hereby incorporated into Permit No. M-2000-115. All
other conditions and requirements of Permit No. M-2000-115 remain in full force and effect.
The estimated liability amount of $46,503.00 exceeds the $41,600.00 performance bond currently held. If you have not
already done so, please submit additional bond in the amount of $4,903.00. The revision will not be final until the bond
is approved by the Division.
