This report is to be submitted within 30 <br />days after the end of the period of exploration specified in the approved Notice of Intent, as <br />amended. <br />1. Attach a map showing: <br />a) The final locations of all holes drilled during this project; <br />b) the locations of all roads modified or constructed for the project; and <br />c) All areas significantly disturbed by the operator. <br />2. Have all disturbed areas, including drill holes, been reclaimed in accordance <br />with the approved plan and the above-referenced regulations? Yes <br />3. Were any newly constructed project roads retained for the landowner? No <br />If yes, a) attach a letter from the landowner that grants consent, b) indicate the <br />road location on a map, and c) describe the approximate specifications of the <br />road (width, length, surface material used). <br />4. Did any drill holes experience surface artesian flow conditions? =-cNo <br />If yes, locate these holes on a map and describe sealing and plugging procedures <br />that were used. <br />5. Were any drill holes converted to a water well? No If yes, list the State <br />Engineer's Well Permit Number and name and address of the user. <br />Nine holes were converted to monitoring wells, but not <br />production wells. SEE ATTACHED
