Dear Mr. Hedrick:
On February 19, 2009, the Division of Reclamation, Mining and Safety received three revised originals of a Transfer of
Permit and Succession of Operators Application, three revised originals of a Performance Warranty and a One Site
Exploration Financial Warranty Corporate Surety, in the amount of $20,600.00, for Agri-cal #1, Permit No. M-1997-064.
The Division is returning these documents for the following:
Succession of Operators Application:
The Division requires the successor operator's company name be listed within the Notary for Successor section on
page 5. Please have your notary list "GCC Rio Grande, Inc." rather than "Manufacturing & Operations".
Performance Warranty:
The Division requires the company name listed within the Notarization of Operator's Acknowledgement on page 3
match the company name listed with the State of Colorado Secretary of State's Office (i.e."GCC Rio Grande, Inc."
rather than "GCC Rio Grande Inc."). Please have your notary make and initial this correction.
One Site Exploration Financial Warranty Corporate Surety
The One Site Exploration Financial Warranty Corporate Surety form is intended for use with Notices of Intent,
rather than permitted operations. The appropriate corporate surety form for this site is the Financial Warranty
Corporate Surety form. Please find a Financial Warranty Corporate Surety form enclosed for your use.
Upon correction, please return only one, rather than three, of each of these documents to my attention.
Please note that the Division requires only one original Succession of Operators Application and one Performance Warranty.
Therefore, the Division is returning the two extra originals of each of these documents.
