Dear Mr. Hedrick:
On January 21, 2009, the Division of Reclamation, Mining and Safety received three originals of a Transfer of Permit and
Succession of Operators Application, a $144.00 Succession of Operators application fee and three originals of a Performance
Warranty for Agri-cal # 1, Permit No. M-1997-064. The Division is returning the Succession of Operators Application and
the Performance Warranty for the following:
Succession of Operators Application:
The Division requires a financial warranty be submitted with this application when a successor operator initials
Option B on page 1. If you do not intend to submit a financial warranty at this time, please initial Option A. If you
do intend to submit a financial warranty with the submittal of this application, please send it to my attention.
The Division requires the current permittee's company name be listed as Permittee at the top of page 5. Please list
"Western Nutrients Company, Inc." as Permittee rather than "R.G. Screws".
The Division requires completion of the Notary for Permittee section on page 5. Please have your notary list
"Western Nutrients Company, Inc." as the company for which R.G. Screws is Secretary.
The Division requires the successor operator's company name be listed as Successor Operator at the top of page 5.
Please list "GCC Rio Grande, Inc" as Successor Operator rather than "Ronald Hedrick".
The Division requires the successor operator's company name be listed within the Notary for Successor section on
page 5. Please have your notary list "GCC Rio Grande, Inc." rather than "Manufacturing & Operations".
Performance Warranty:
The Division requires a corporate seal be affixed where indicated on page 3.
