This report covers the time period from October 1, 2008 to December 31, <br />2008. The Impoundment Location Map details the location of each impoundment inspected <br />' (Attachment A). Quarterly Pond Inspection Reports document water level, sediment level, <br />outflow, and required maintenance cited per inspection (Attachment B). <br />' The inspections were performed on December 2, 2008 by Field Inspector Chris Miller. The site <br />was covered with snow. All structures were functioning as designed at inspection.
