This report constitutes the sixth such document <br />generated to date. The report addresses on-site inspections occurring November 28, and December 1, 8, <br />20, 21, 27, and 28. This report is submitted to fulfill the quarterly inspection report for the fourth quarter <br />as specified by Rules 4.09.1(11)(x) and 4.10.2(2)(a). Compaction testing results from September and <br />October 1995 are appended to this report. <br />I apologize for the delay in forwarding this information to you. Although the report was available at the <br />mine office, I neglected to forward a copy to the Division office. Thank you for your cooperation in this <br />matter.
