SNEITON, Dlnetor Blue Jet Mining Inc Box 181 Craig, CO 81625 110 Annual Fee Letter August 29, 1986 SEP 1 ~ 1986 GKra~3 Section 34-32-110(4) of the Colorarto Mined Land Reclamation Act requires that, "...the Operator shall annually file, on the anniversary date of the permit, a Notice of Intent to Continue Mining Operations, The Notice of Intent shall contain an annual fee of E50.00 and a map or statement that shall indicate: (a) the land affected during the previous year; (b) the land to be affected during the coming year; and (c) any land reclaimed during the preceding year." Division records indicate you owe E50, Permit No.: M-78-135NR Operation flame: Blue Jet Mine Anniversary Date: 09/09/86 Annual Fees for the Year(s): 1986 Total: E50 (Due on the Anniversary Date) If your records indicate different facts, please inform this office as soon as possibie; otherwise, we will expect to receive the annual fees as required by the statute on or before your anniversary date. Also, please include the required Notice of Intent to Continue Mining Operations (Annual Report) to bring the Division up-to-date as to the progress of mining and reclamation on tfte site. If you cannot include these materials with the fees, please indicate on the enclosed sheet how soon we should expect to receive the material.
