Please note that no original annual <br />xepoxt form was ever received from the Division. When I called about that I was informed that your server <br />had failed at one point and there were a number of annual report requests that had never been sent out and <br />they would be sent later. No forth was ever received, but the annual report was nevertheless provided on <br />May 18, 2005 along with the payment. <br />Please update your records to indicate that the annual xepoxt was provided and the required fee paid.
