Cattany <br />Division Director <br />Natural Resource Trustee <br />On December 27, 2004, the Division of Minerals and Geology received a check in the amount of $281.00 and <br />annual report for the Perdue Pit, Permit No. M- 2000-089 in accordance with C,R.S. 34-32.5-116 or C.R.S. <br />34-32-116 of the Constmcfion Materials Rules and Regulations. The Division regrets to inform you that check <br />no. 3259 was returned to the Division due to non-sufficient funds. Anon-sufficient funds fee in the amount of <br />$25.00 has been assessed. <br />Therefore, please send either a cashier's check or money order in the amount of $306.00 to the Division on or <br />before February 21, 2005. <br />Please be advised that if the Annual Fee and Non-sufficient Funds fee aze not received by February 21, 2005, a <br />Forma] Notice of Violation and a Board Order to pay civil penalties in the amotmt equal to the annual fee will <br />be automatically assessed.
