In order to maintain your mining operation in compliance with the 1a the
annual fees, report, and map required by CRS 34-32-116(1)(a) must be submitted
annually on or before your permit anniversary date. Fees are based pon a
schedule of 5350.00 if the operation is an open mining operation andE275.00
for any other operation. Division records indicate you owe E350.

the statute on or before your anniversary date. Also, please includ the
required Notice of Intent to Continue Mining Operations (Annual Repo t) to
bring the Division up-to-date as to the progress of mining and recla anon on
the site. If you cannot include these materials with the fees, plea e
incidate on the enclosed sheet how soon we should expect to receive ~he
material.
Submittal of a report and map (to a scale of not less than 100' to t~le inch
and not more than 660' to the inch), indicating the phases of the re lamation
plan which have been completed and correlated with the timetable is ~equired.
The report and map shall contain the following information:
1) Location and number of acres affected during that year,
2) Locations and number of acres reclaimed for that year,
3) The location and number of acres in the various stages of I
reclamation (backfilled, graded, topsoil replaced, revegetated),
4) The species of vegetation planted, the iocations and appro}cimate
