111 Annual Fee Letter

By way of reminder, a 111 permit (Special Permit) such as yours is for mining
which must occur within a specifically short time. If you wish to
continuously mine the site, you should re-apply for a 110 (less than 10 acres
and 70,000 tons per year), or a 112 permit (if you wish to mine more than 10
acres or 70,000 tons per year). The annual fee and report are required
pursuant to 34-32-116(1)(a) of the Colorado Mined Land Reclamation 11ct,
C,R,S., as amended. The fee is based upon a schedule of E350:00 if the
operation is an open mining operation and 5275.00 for any other operation.

1} The location and number of acres in the various stages of
reclamation (backfilled, graded, topsoil replaced, revegetated),
2) The species of vegetation planted, the locations and appro:imate
dates of planting,
3) The type and approximate quantity of fertilizers, organic material
or soil conditioners used and location of use,
