We have not received the annual fees and report. Therefore, a violation exists to Permit No. M-77-132- and a civil penalty has been assessed in the amount of E350.00, due and payable within 30 days of the date of this letter, If the annual fees, annual report and the civil penalties have not been submitted within 30 days of this notice, March 5, 1990, this permit will be scheduled for Formal Public Hearing before the Mined Land Reclamation Board for consideration of permit revocation and forfeiture of the 533,500.00 financial warranty at the March Board Meeting. The March Board Meeting will be held on March 21 and 22, 1990, in Room 220 of 1313 Sherman Street, Denver, Colorado, beginning at 9:00 a.m. or as soon thereafter as the matter can be Considered and you will have the opportunity to be represented and heard.
