In the <br />review, the Division posed specific questions which required a <br />response. The Division requested that the responses be received by <br />no later than June 11, 1996. To date, no response has been <br />received by the Division. <br />Some of the questions posed by the Division in its review concerned <br />whether or not monitoring had been conducted in accordance with the <br />approved monitoring plan. A copy of the review is attached. <br />Please submit a response to the review by July 8, 1996. <br />If no response is received by that date, the Division will continue <br />its review based on the information provided in the report. From <br />the report, it appears that monitoring was not conducted in <br />accordance with the approved monitoring plan.
