Fee and Report, Notice
C.R.S. 34-32-116(3) and (4), 1989 Supp. and Permit No. M-84-013 require
the Division of Minerals and Geology $225.00 in annual fees and an annua
• before May 14th of each year. As of the date of this letter the Division has
annual fee and report for May 14, 1995. Therefore, you are in Violation of
statute and the provisions of your Permit No. M-84-013. Pursuant to C.R.S.
Mined Land Reclamation Board hereby assesses Notice of Violation No. M--
Penalties in the amount of $225.00, and Notice and Order to Cease and De
mining related activities associated with Permit No. M-84-013.
NOTE: NO PERMITS FOR NEW MINING OPERATIONS SHALL BE G
OPERATOR WHO IS CURRENTLY FOUND TO BE IN VIOLATION IN
(C. R.S. 34-32-120) UNTIL THE VIOLATION IS ABATED.
If you fail to submit your annual fee, annual report, and the Civil Penalties
Division by October 18, 1995, this matter will be heard at a Formal Public
Mined Land Reclamation Board for consideration of permit revocation, forfl

October 19, 1995 Board Meeting. If you submit all past due annual fees, r
