B sed upon our records, the anniversary date of your mining operation permit is
~anuary 23 and annual fees for the year(s) 1983 totalling
$ are due on that date. Fees are based upon a schedule of
$350.00 if the operation is an open mining operation of 10 acres or more and
$275.00 for any other operation 10 acres or more.
. .~Zso, to bring the Division up to date as to the mining and reclamation on the
site, please submit a report and map (to a scale of not less than 100' to the
inch and not more than 660' to the inch), indicating the phases of the reclama-
tion plan which have been completed, correlated with the timetable. The report
and map shall contain the following information:
1) Location and number of acres affected during that year,
2) Locations and number of acres reclaimed for that year,
3) The location and number of acres in the various stages of reclamation
(backfilled, graded, topsoil replaced, revegetated),
4) The species of vegetation planted, the locations and approximate dates
of planting,
5) The type and approximate quantity of fertilizers, organic material or
soil conditioners used and location of use,
6) An appraisal of the success of reclamation efforts, and in the event
of failure of any efforts, a short narrative on the suspected or determined
causes of such failure,
7) Estimates of the Location and number of acres to be affected in the
coming year, and
8) The effective date of the information provided above shall be no more
than 45 days prior to the anniversary date.
