P,.S, i973, as amended. <br />Please send the following (indicated by check marY,) vi thin 30 days: <br />Annual fees in the amount o_° 5 350.00 Plake check or money order <br />payable to the Colorado alined Land Reclamation Division. <br />Annual Report describing: <br />I) The location and number of acres in the various stages of reclama- <br />tion (backfilled, graded, topsoil replaced, revegetated), <br />2) The species of vegetation planted, the locations and aporoximat_° <br />dates of an ring, <br />3) The type and approximate quantity of fertilizers, organic <br />material or soil conditioners used and location of use, <br />4)~ An appraisal of the success of reclamation efforts, including <br />locations' and number of acres reclaimed for that year, and in the event of failure <br />of any efforts, a short narrative on the suspected or determined causes of <br />such failure, and <br />5) The effective date of the information provided above snail be no <br />more than 45 days prior to the anniversary date. <br />
