The annual <br />report. sent with your fee was insufficient to determine what, if any, mining <br />or reclamation has taken place in the past year, Submission of your annual <br />report. is required by the Mined Lano Reclamation Act C, R. S, 34-32-116(3), and <br />by Rules and Regulations Rule 2.4.1 for 112 Regular Operations. <br />2.4,1 Annual Report <br />On the anniversary date of the permit each year, or upon a specific date <br />if requested by an operator for the purpose of submitting annual reports <br />for more than one permit on one date, the operator shall submit a report <br />and a map showing the extent of current disturbances to affected land, <br />reclamation accomplished to date and during the preceding year, new <br />disturbances that are anticipated to occur during the upcoming year, and <br />reclamation that will be performed during the upcoming year. If no new <br />disturbances or reclamation have occurred during the previous year and no <br />changes to the previous year's map are necessary, then no new map is <br />required, provided that the operator shall state this in the annual <br />r•epor t. <br />P1easE~ till in the information on the enclosed Notice of Intent to Continue <br />Mining Operations 112 Annual Report form and return to the Division, Also, <br />reviev your Permit document for any special reports, information, or data <br />sub~nii:tal which may be required as part of the annual report. <br />If an annual report is not received within 60 days of the date of this letter, <br />an inspection of the site of this operation will be scheduled to assess <br />compliance.
