However, the required $150.00 revision application fee for regular 112 operations was <br />not provided with the request. Therefore, the application is incomplete and can not be <br />considered at this time. Please submit the $] 50.00 application fee to our Durango Office as soon <br />as possible so that we can begin the review process. The Division will review the application for' <br />technical adequacy within 30 days of receipt of the required fee.
