Lamm Governor 112 Annual Fee Letter August 1, 1984 In order to maintain your mining operation in compliance with the law, the annual fees, report, and map required by C.R.S. 34-32-116(1)(a) must be submitted annually on or before your permit anniversary date. Also, please include the required Notice of Intent to Continue Ftintng Operations (Annual Report) to bring the Dl vision up-to-date as to the progress of mining and reclamation on the site. If you cannot include these materials with the fees, please tnctdate on the attached sheet how soon we should expect to receive the material. Submittal of a report and map (to a scale of not less than 100' to the loch _ and not more than 660' to the inch), indicating the phases of the reclamation plan which have been completed and correlated with the timetable is required.
