PLEASE READ CAREFULLY
Under the terms of your Reclamation Permit and Colorado Statutes, you must submit Annua]
Fees and Annual Reports. The Division has received your annual fee and report. Please be
advised that due to the annual fee increase that went into effect July 1, 2007 you have a remaining
balance due. The Division is aware that you have complied with and sent the fee stated on your
annual fee and report request letter, but a balance of $103.00 is still needed. Please pay
remaining balance by close of business on August 31, 2007. The Division apologizes for the
inconvenience and thanks you for your understanding.
PLEASE NOTE: If the remaining annual fee balance is not received on or by August 31, 2007,
a formal notice of violation and a board order to pay civil penalties in the amount equal to the
annual fee will be automatically assessed. No permits for the new mining operations shall be
granted to any operator who is currently found to be in violation in this state (C.R.S. 34-32.5-120
OR C.R.S. 34-32-120) until the violation is abated.
Division records indicate $ 103.00 fee is due. Thepivision has received a total of $ 688.00.
Please send a CHECK or MONEY ORDER made payable to the Division of Reclamation,
Mining and Safety, 1313 Sherman St., Room 215, Denver, Colorado 80203.
