Long Division Director Rule 2.04.13(1):By February IS, or other such date as agreed on, each permittee shall file an annual reclamation report covering the previous calendar year for all areas under bond. The report shall include, but not be limited to, text, discussion and maps which address: The report is in compliance with the date of receipt. It was received at the Division on February 11, 2000. Rule 2.04.13(1)(a): The name and address of the permittee and permit number; The report is in compliance with this rule. The name and address of the pemuttee and permit number are NCIG Financial, Inc. % J. E. Stover & Associates, 2768 Compass Drive, Suite 101, Grand Junction, CO 81506. The permit number is C-84-065. Rule 2.04.13(1)(b): Location and number of acres disturbed during that year; The report is in compliance with this rule. No land was disturbed during 2001. Rule 2.04.13(1)(c): Location and number of acres bacl~lled and graded during that year; The report is in compliance with this rule. No land was backfilled or graded during 2001. Rule 2.04.13(1)(d): Location and number acres topsoiled during that year; The report is in compliance with this rule. No land was topsoiled during 2001. Rule 2.04.13(1)(e): The species, location and number of acres of vegetation planted during that year, including any augmented seeding or cultural practices; and The report is in compliance with this rule. No land was seeded or cultivated during 2001.
