The application cannot be approved until the following remaining issues are adequately <br />addressed. Please submit a response to the following issues as soon as possible, but no later than May 12, <br />2004 so the decision due date set for May 14, 2004 can be met. If you cannot respond by May 12, 2004 <br />please submit a written request for extension of the decision date. If your response, or an extension request, <br />are not received by May 12, 2004 this application may be denied. <br />ADEQUACY ISSUES <br />Exhibit A -Legal Description <br />1. Response acceptable. <br />Exhibit B -Site Description <br />2. The list of structures and their owners notes that the small structure just east of the pump house <br />owned by Dustman is a shed. This structure is actually an old outhouse that does not appear to <br />have been used in quite some time. However, it still qualifies as a structure to be protected. <br />Additionally, there are fences on the Carlill property that are located within 200 feet of the <br />proposed permit area. These fences are shown on Map B-1. Please include these fences in <br />Exhibit L. <br />3. Response acceptable. <br />4. Response acceptable. <br />5. The third paragraph on page 16 is inconsistent with the second paragraph, in that it specifies <br />topsoil piles with sides no steeper than 2:1 rather than the recommended 3:1 grade.
