As staff reviewed the file it was discovered that this Office had an additional set of maps that goes with the
supplemental information submitted by Lewicki and Associates. Staff thought it might be helpful to include
those as well. These documents do not cover the entire file since the Board Order approving the permit in
December of 2003. However, they do represent the important aspects of the on going issue before the
Board. Anyone wishing to review the entire file or obtain copies of these documents may make a request to
