Preliminary review of the information received <br />determined that the following items must be received before the Division can consider your application as being <br />submitted and technical review can begin: <br />APPLICATION FEE: The Division did not receive entire application fee of $1,094.00 with the application <br />package. Please remit an additional $219.00. See new forms enclosed <br />APPLICATION FORM -item 3 -Permitted acreat=_e (new or ezistioH site): the acreage for a Construction <br />Materials Limited Impact (l l0) Operation is limited to 9.99 acres. Please make the necessary corrections to <br />Item 3 and any other erdtibits where you have 10 acres listed i.e. maps, legal description, etc. <br />Aoolicatioo Sittnature Paee -Pace 6: Applicant/Operator should be 3B Enterprises. <br />Application Siznature Paee -Paee 6: Notarization of your signa[tre should read: <br />"The foregoing instrument was acknowledged before me this I" day of June 2001 , by _ <br />Steve Baker as Owner of 3B Entemrises <br />EXH~IT A -Leval Description (Rule 6.3.1(1)): The legal description must identify the affected land <br />specify affected areas and be adequate to field locate the property and be limited to 9.99 acres. Description <br />shall be by township, range, and section, to at least the nearest quarteryuarter section. A metes and bounds <br />survey description is also acceptable. Also, the street address or lot number(s) may be used. This information <br />is available from the County Assessor's office or U.S. Geological Survey (USGS) maps. <br />EXHIBIT J -Proof of Mailin~Notices of Permit Aoplication~ule 6.3.10): Include proof of mailings of <br />[he "Notice of Permit Application" sent to the Board of Supervisors of the Colorado First Soil Conservation <br />District, in the application, by submitting the date-stamped receipt far Cert~ed Mailing or adate-stamped <br />copy of the notice form with signature acknowledging personal service and receipt by the appropriate board. <br />Also, refer to Rule 1.6.2(1)(x). <br />
