On November 29, 2001, the Division of Minerals and Geology received your the 110 Construction Materials
Reclamation Permit Application package for the Starks Quarry, File No. M-2001-114. Prelimiuary review of the
information received determined that the following items must be received before the Division can consider your
application as being submitted and technical review can begin:
APPLICATION FORM: Item 1, Page 1 on the Application Form Lists the Applicantl0perator Name as
Loukonen Bros. Stone. Item 14, Page 3 on the Application Form fists the Applicant/Operator Company
Name as Loukmren Bros. Stone. Page 6 shows the Applicant/Operator name listed as Loukonen Bros.
Stone. The Corporate Seal on Page 6 shows the Corporation Name as Loukonen Bros. Stone Company.
The notary notarized Michael Loukonen's signature as General Manager of Loukonen Stone Co. Exhibit
G -Legal Right-to-Emer shows that the lease is between Douglas E. Starks and Loukonen Bros. Stone
Company. The Applicant/Operator name must be consistent throughout the entire applicafion
documentation. Please make all of the necessary changes to the Applicant/Operator name (your original
application form is enclosed so that you can make the changes, you will need to send replacement pages
for any exhibits that need to be corrected).
APPLICATION FORM -Item 9 -General Location Information: The Division request a complete
location description be given for Item. 9 of the Application form. The information that was enclosed in the
form submitted only states that the site is 1 mile east. It doesn't state the closest Town or Municipality.
EXHHIBIT E -Map (Rule 6.3.5): The only map that was enclosed for this exhibit was a location map.
(1) In addition to the requireme~s of 6.2.1(2), you must provide a map that clearly describes the features
associated with the mining plan and the components of the Reclamation Plan. Include one map for the
mine plan and one map for the Reclamation Plan. The map(s) must be drawn to a state no smaller than
appropriate to clearly show all etemems that are required to be delineated by the Act and these Rules;
show a north arrow, note any section comers adjacent to the proposed operation, and indicate the date
illustrated. At a minimum, maps must include the following information:
