The conditions to the approval are <br />noted below: <br />Stipulation No. O1 <br />The operator shall submit to the Division plans for containment of all <br />hydrocarbons used, generated, and/or stored on site. Plans shall be <br />consistent with the requirements of 34-32-116(1)(h). This plan must he <br />obtained from the Eagle County Health Department and the Mined Land <br />Reclamation Board prior to construction of containment facilities and <br />prior to the presence of hydrocarbons at the site. The plan shall <br />consist of two parts: a) a plan for fuel storage, spill prevention, and <br />spill containment; and, b) a plan for handling, storage, and disposal <br />of all spent petroleum products. <br />Stipulation No. 02. <br />The operator shall submit to the Division plans for relocation of the <br />CKP ditch. Plans shall be consistent with the requirements of <br />34-32-116(1)(h). These plans shall be approved by the Mined Land <br />Reclamation Board prior to the excavation of the existing ditch. <br />The amount of financial warranty set by the Roard for this operation is <br />$14,950.00. You must submit a financial warranty in this amount and a <br />performance warranty in order for us to issue a permit. In the event you have <br />requested a financial warranty form, we have enclosed it in this letter. If <br />you have not, please select a type of financial warranty from Rule 7.4. Then <br />contact us so that we can provide you with the appropriate warranty form.
