Materials Reclamation Permit Application Package
Dear Mr. Allen:
On August 14, 2000, the Division of Minerals and Geology received your the 110 Construction Materials
Reclamation Permit Apphcation package for the A.P.S. Trucking and Excavating, Inc., File No. M-2000-103.
Preliminary review of the infomlation received determined that the following items must be received before the
Division can consider your application as being submitted and technical review can begin:
APPLICATION FORM - Company Name: The Applicant/Operator or Company name to be used
on the permit is listed two different ways: 1) Allen P. Small Trucking & Excav. Inc. 2) A.P.S.
Trucking and Excavation, Inc. Also, the Corporate Seal for this company is shown as Allen P. Small
Trucking 8t Excavation, Inc. The name of the company must be consistent throughout the application
and on all other legal documents. Mr. Small's company is registered with the Secretary of State's
office as "Allen P. Small Trucking & Excavation, Inc.". This is the name that you need to use on the
application form, bond and Performance Warranty. The application pages that need to be replaced are
Page 1, Page 3, and Page 6. Wherever the name of the "ApplicanUOperator" or Company is requested
you need to change the name to "Allen P. Small Trucking & Excavation, Inc.".
EXHIBIT [ -Proof of Filine with the County Clerk: The Division requests verification of this
exhibit. It is unclear who the person is that initialed page I of the application, that is being submitted
as Exhibit I.
EXHIBIT J -Proof of Mailine Notices of Permit Application (Rule 6.3.101: Include proof of
mailings of the "Notice of Permit Application" sent to the Board of Supervisors of the local Soil
Conservation District, in the application, by submitting the date-stamped receipt for Certified Mailing
or adate-stamped copy of the notice form with signature acknowledging personal service and receipt
by the appropriate board. Also, refer to Rule 1.6.2(1)(a). If the NRCS is signing on behalf of the
SCD they need to state this on the copy of the application they signed.
Your application will not be considered filed until the information listed above is received and found sufficient
to begin our review. A decision date will be established 30 days from the date of receipt of all of the requested
information. Additionally, if you have already published notice you will need to republish notice, after the
