Application
Dear Mr. Ullom:
On June 8, 1995, the Division of Minerals and Geology received your application package for the
Johnson Draw, File No. M-95-046. Preliminary review determined that the following items must be
submitted before your application can be considered submitted and technical review can begin:
EXHIBIT O -Proof of Marline Notice of Permit Aoolication (Rule 6.4.171: You must submit
actual delivery or proof of mailing by Certified Mail, return receipt requested, the "Notice of
Application" forms to the Rio Blanco Board of Counry Commissioners and to the Douglas Creek
Soil Conservation Disctict. Exhibit I indicates that you requested information from the Soil
Conservation Service sometime around April 1, 1994. Exhibit M shows your application to the
Colorado State Health Dept for an Emission Permit. You must submit proof of notification to the
County Commissioners and the SCD pertaining the filing of the Reclamation Permit Application
that you sent to the Division in June, 1995.
Rule 1.6.2f1)(b) (Effective May 1. 1995): You must post a notice at the location of the proposed
mine site of sufficient size and number to clearly identify the site as the location of a proposed
mining operation giving your name, address, and phone number, and stating that you have applied
for a mining permit with the Colorado Mined Land Reclamation Board. Your notice must also
state that anyone wishing to comment on the application may view the application at the Adams
County Clerk and Recorder's Office and should send comments prior to the end of the public
comment period to the Colorado Mined Land Reclamation Division, Room 215, 1313 Sherman
St, Denver, Colorado 80203.
After posting this notice you must provide a signed affidavit that such notice has been posted
according to the provisions of this Rule and submit the affidavit to the Division.
A sample of this notice and affidavit is enclosed for your use. Please be sure to place a copy of
the affidavit at the Clerk and Recorder's office.
