Watcher <br />Executive Director <br />Michael B. Long <br />Division Director <br />This Office has conducted a Preliminazy Adequacy Review of the above referenced application. The <br />decision date for this application has been set for June 24`s, 2002. A reclamation cost estimate will be <br />undertaken as soon as all issues aze abated. Please find listed below the issues that staff have concluded <br />need to be corrected or clarified before the application can be approved. <br />1) The supplied map(s) does (do) not have the required signature of the qualified person who prepazed i[ <br />(them) as required by Rule 6.2.1(2)(b). Please supply one original and two copies of a corrected <br />version(s) of the map(s) with signature and any other issues noted in the application review process. <br />2) Exhibit "C"-1 has a property identified on the map as #89 and as #87 of [he index. Please correct on <br />new maps. <br />3) Exhibit "C"-5 shows a building or structure partially in the sewer line buffer zone and the rest within <br />the disturbance. Within the Mining Plan, please clarify the disposition of this and like structures noted <br />on the maps as within the disturbance area. Also, please commit to reclaiming buffer azea disturbances <br />if impacted by mining activities. <br />4) Reclamation Maps and Plan note topsoil on slopes to water level. However no mention or map <br />reference is made of reclamation of flat areas surrounding the pond. Please clarify and commit to <br />reclamation of these azeas. <br />5) Exhibit "O" list Terry Deherra as an adjacent landowner. This landowner is not identified in Exhibit <br />"C"-6. Also missing from the list of adjacent landowners is Miguel Flores and the 401 30 Road <br />Partnership. Please clarify and make appropriate adjustments to the exhibits. <br />6) In Exhibit "S"-2, the index of permanent structures within the mining azea on "S"-1 are not <br />identified. This relates to #3 above. Please identify the structures on the map and in the index. <br />
