Therefore, enclosed please find two signed original permits for this operation. Please also sign the rider page on each permit. Then, retain one permit for your files and return the other to us within two weeks. Please be advised that all of your original application ma.teriais, as well as any and all adequacy submissions by your consultant, are considered an integral part of your permit and are incorporated by reference. It is presumed that you have a copy Gf all of these materials; therefore, none is enclosed with this mailing. Be further advised that it is your responsibility as a permit holder to submit each year on or before the anniversary date of your permit, a 550.00 annual fee and an annual report. Although we wil] attempt to notify you of this requirement, it is your responsibility tc submit these items. Finally, I suggest you read the rider page of the permit document and take note of the two permit stipulations on that page. Both stipulations require further action on your part.
