Initial review <br />determinai that the following items must be submitted before technical revie~d <br />of your application can begin: <br />EXHIBIT R - Proof of Mailing of Notices to County Commissioners and Soil <br />Conservation District: Proof of actual delivery or proof of mailing by <br />certified mail, return receipt requested, of notice of the application to <br />the local board of county commissioners, and if the mining operation is <br />within the boundaries of a soil conservation district, to the board of <br />supervisors of the soil conservation district. We suggest if you <br />hand-deliver the notices that you have the commissioners and the SCD <br />clerk date-stamp and sign the notice and give you a copy of the notice <br />for the application package. <br />EXHIBIT S - Proof of Filing with County Clerk and Recorder: Please <br />submit an affidavit or receipt indicating the date on which the <br />application was placed with the local County Clerk and Recorder. <br />Failure to submit the items listed above by the initial decision date of <br />September 30, 1992, will result in a decision to deny your application. You <br />may amend your application by submitting the requested items. In accordance <br />with Rule 1.5, a new decision date will be established 90 days from the date <br />of receipt of all requested items. If ycu have already initiated publication, <br />you will need to republish notice which specifies the final date for receiving <br />comments is the 20th day after the fourth publication or the next regular <br />business day. <br />
